For issuing of donation receipt, please return the direct bank transfer note with this completed "Donation Form" by fax, email or WahtsApp to the Centre. 為發出捐款收據，請將存款收條連同此"捐款表"以傳真/電郵/郵遞或WhatsApp到努力試中心。

Please "Submit Donation Form" (by pressing the Green button) and then "Make Credit Card Payment" (by pressing the Orange button) 請"送出捐款表" (按線色), 然後進行"信用卡付款"(按橙色)。
